What P0721 means
The output shaft speed sensor (Hall or inductive) reports the speed of the transmission output; it gives the gear ratio together with input speed and on many vehicles also the road speed for speedometer and cruise control.
How the engine computer detects it
P0721 means the output speed is present but implausible: too high (Ford 6R80 above 7450 rpm), not matching wheel or input speed in the engaged gear (ZF 6HP26 more than 200 or 250 rpm apart, Ford's F21 more than 15 % for 10 s), or jumping (Allison 1000: 500 rpm within 0.15 s). On GM's 10-speed with a direction-sensing sensor, P0721 means the reported direction of rotation does not fit – since 2025 after two trips, earlier after one. Wrong tire sizes or a faulty wheel speed sensor can also falsify the comparison.

Symptoms
- Speedometer jumps
- Warning lamp
- Wrong shifts, limp mode
- ABS/ESP warning possible if wheel speed is affected
Common causes
- Output speed sensor faulty or dirty (metal debris) Most likely
- Contact problems or interference in the sensor circuit Possible
- Wrong tire size or axle ratio, faulty wheel speed sensor Possible
- Reluctor wheel damaged or sensor mounted wrongly Less likely
How to diagnose P0721
- Read every stored code with an OBD-II scanner. If misfire or fuel trim codes are there too, fix those first: they often cause this one.
- Inspect the parts involved: output speed sensor, sensor wiring, transmission control module.
- Work through the causes above from most to least likely, cheapest checks first.
- Clear the code and drive a full drive cycle to confirm the repair held.

Counts of owner complaints to the NHTSA that mention P0721 (25 in total), not failure rates: popular models get more complaints simply because more of them are on the road. Models with fewer than 3 complaints are left out.

Questions drivers ask
Can I drive with code P0721?
Drive gently and only as far as you need to. The engine computer may limit power to protect the engine, so avoid highways and heavy traffic until it's fixed. If the check engine light starts flashing, pull over.
Will my car pass an emissions inspection with P0721?
Probably not in states that test vehicles with an OBD-II scan: an emissions-related code that keeps the check engine light on is usually an automatic fail. Rules differ by state and county, so check yours.
Will P0721 clear by itself?
The light can go off after several drive cycles without the fault, but the code returns if the cause is still there. After a repair, the readiness monitors need a full drive cycle before an inspection.
Can I fix P0721 myself?
Often yes, with an OBD-II scanner and basic tools. Difficulty: medium. Start with the cheapest checks before replacing expensive parts.
Often reported together
Codes that owners mention in the same NHTSA complaints as P0721. They can point to a shared cause, so it's worth checking them together.
